Landmark OPTIMA trial delivers practice-changing evidence that the Prosigna test identifies patients with high-risk breast cancer who can safely avoid chemotherapy

Prof. Robert Stein, Chief Investigator, University College London, presented the OPTIMA trial results at the ASCO 2026 conference.

Treating breast cancer requires solving a complex equation

The Prosigna test is the only test proven to be predictive for chemotherapy treatment decisions for patients with up to nine positive nodes.5

  • The Prosigna test is based on the PAM50 genomic classifier, a foundation of breast cancer biology.
  • The Prosigna test is for patients diagnosed with early-stage hormone-receptor positive (HR+) breast cancer.
    • Performed on existing tissue samples obtained during a core needle biopsy or surgical resection, prior to any treatment.
    • Determines a patient’s Risk of Recurrence (ROR) score and associated 10-year probability of distant recurrence.
    • Identifies patients who may benefit from chemotherapy and those who may safely avoid it.

The complete formula

The Prosigna Breast ROR test factors a patient’s biological, clinical, and pathological information into a comprehensive report that includes:

  • Intrinsic subtype: classified as Luminal A, Luminal B, Basal-like or HER2-enriched.
  • Prosigna Risk of Recurrence (ROR) score: ranges from 0-100.
  • 10-year probability of distant recurrence with 5 years of endocrine therapy.
  • Predicted chemotherapy benefit when the Prosigna ROR score is >60.

Behind every breast cancer treatment decision is a patient asking, ‘Do I really need chemotherapy?’

OPTIMA answers this question with robust evidence. These study results can be practice-changing – we can now use the Prosigna test to predict which high-risk patients can safely avoid chemotherapy and its lasting side effects without compromising outcomes. This is a profound shift for patients and the clinicians who care for them.”6
